Heart failure is the terminal stage of a variety of chronic heart diseases and has a high incidence. Modern doctors often approach it from the perspective of the spleen and stomach and can achieve good results in treating heart failure. In order to understand the advantages of treating heart failure from the spleen, based on the central theory of the spleen and stomach and other scientific theories of traditional Chinese medicine, this paper expounds the mechanism of dialectically treating heart failure from the spleen and reviews relevant information in recent years. In the literature, through the analysis and summary of the clinical effects of modern doctors treating heart failure from the spleen, from the aspects of traditional Chinese medicine and cardiology, the clinical effects of modern doctors treating heart failure from the spleen are analyzed and summarized. The advantages of treating debilitating diseases, spleen and stomach symptoms and the efficacy indicators of Western medicine are discussed separately. It is found that traditional Chinese medicine is conducive to optimizing the pathogenesis of heart failure and guiding its prevention and care. It is also beneficial to the reduction of spleen and stomach symptoms (digestive system) in heart failure and the improvement of the clinical efficacy indicators of Western medicine. It is hoped that this discussion can provide a guiding basis for the clinical treatment of heart failure from the spleen, enrich the theory of heart failure treatment methods, and bring benefits to patients with heart failure.

In contemporary China, the phenomenon of population ageing is becoming increasingly salient. The rapid growth of the elderly population has led to a concomitant rise in the prominence of the issue of elderly care, which is now a matter of growing public concern. The implementation of the community-based elderly care model plays a pivotal role in effectively responding to the challenges brought about by an ageing society. It can provide more comprehensive and caring elderly care services for the elderly, thereby further promoting the sustainable and sound development of the elderly care cause. Presently, deficiencies persist in the provision of community-based elderly care in China. These include insufficient service supply, imperfect facility construction and a shortage of human resources. To address these issues, it is imperative for the government and all societal sectors to collaborate, refine pertinent policies, facilitate the recruitment of advanced professionals, and implement other strategies. This will ensure that the elderly receive convenient, efficient, and accurate senior care services, allowing them to enjoy a peaceful twilight years.

Chronic diseases, also known as non communicable diseases (NCDs), refer to diseases that have a long course, slow progression, are difficult to cure, and typically require long-term medical care. Common chronic diseases include hypertension, diabetes and stroke. These diseases not only seriously affect the quality of life of patients, but also bring heavy economic burden to families and society. This article will focus on exploring the application and effectiveness of exercise therapy in chronic disease rehabilitation from the perspectives of prevention and rehabilitation.

Objective: To explore and analyze the therapeutic effects of physical factor therapy on chronic diseases. Method: A total of 104 patients with chronic diseases admitted to our hospital from July 2023 to July 2024 were selected as the research subjects. They were divided into a physical group (n=52) and a reference group (n=52) using a double-blind mechanism. The physics group received physical therapy, while the reference group received basic treatment. Compare the treatment efficacy, quality of life, and treatment satisfaction between two groups. Result: The total effective rate of the physics group was significantly higher than that of the reference group (P<0.05), and the difference was statistically significant. Before treatment, there was no statistically significant difference in the quality of life, including social integration, mental state, physiological function, and overall health, between the two groups (P>0.05); After treatment, the physical group showed significantly higher levels of social integration, mental state, physiological function, and overall health compared to the control group (P<0.05). The overall treatment satisfaction of the physics group was significantly higher than that of the reference group (P<0.05), and the difference was statistically significant. Conclusion: Physical factor therapy has significant therapeutic effects on chronic diseases and is worth promoting.

Background: This review examined factors contributing to gastrointestinal health behaviors using the COM-B (capability, opportunity, motivation) model. Methods: Five electronic databases (PubMed, Medline, Cochrane, Web of Science, CINAHL) were searched following PRISMA guidelines, extracting the data and applying the inductive deductive method to map the impact factors to the COM-B model. Results: The COM-B model identified 20 influencing factors mapped to five subcomponents: psychological capability, physical opportunity, social opportunity, reflective motivation, and automatic motivation. Conclusions: Enhancing gastrointestinal health entails considering personal, societal, and environmental considerations.

Objective With the rapid progression of urbanization and technological advancements, firefighters increasingly face complex emergency rescue challenges, making them more susceptible to Post-Traumatic Stress Disorder (PTSD). This study aims to develop an efficient tool to predict and identify PTSD risks among firefighters, ensuring their mental well-being and enhancing rescue efficiency. Methods Detailed data collection and analysis were conducted across multiple fire brigades. The data underwent a series of rigorous preprocessing steps, including cleaning, normalization, and feature importance screening. The SMOTE technique was employed to enhance sample balance. Subsequently, we constructed a predictive model based on the Stacking strategy, integrating multiple algorithms such as Random Forests, Gradient Boosting, Support Vector Machines, and k-Nearest Neighbors. Results The model consistently exhibited outstanding performance in a series of validation tests. Its overall accuracy reached an impressive 96%, with F1 scores of 0.94 and 0.98 for non-PTSD and PTSD categories, respectively. Conclusion We have successfully designed a highly precise PTSD risk prediction model for the fire safety domain. This not only aids in bolstering the psychological support for firefighters but also offers valuable insights for mental health research and public health policy formulation. This study hopes to provide value for both academic research and practical applications.

This paper discusses the latest therapeutic developments and comprehensive approaches to treating IgA nephropathy (IgAN), the most common form of primary glomerulonephritis globally.   The review begins by outlining the pathogenesis of IgAN, emphasizing the "multi-hit" hypothesis, where aberrantly glycosylated IgA1 (Gd-IgA1) molecules, autoantibodies, and immune complex deposition in the kidneys drive disease progression. Current treatment strategies focus on supportive care, including blood pressure control and proteinuria reduction using renin-angiotensin system (RAS) blockers.   Newer therapeutic approaches are highlighted, such as targeted-release formulations like TRF-budesonide, which delivers corticosteroids to gut-associated lymphoid tissue to minimize systemic side effects.   Immunosuppressive treatments, including systemic corticosteroids, mycophenolate mofetil (MMF), and emerging therapies like complement inhibitors (e.g., Eculizumab, Ravulizumab), are explored for their efficacy and safety. B-cell and plasma cell-targeting therapies, complement inhibitors, and personalized medicine approaches represent emerging treatment avenues.   Combination therapies, such as corticosteroids with immunosuppressants, are also discussed, aiming to enhance treatment efficacy while minimizing adverse effects.   Personalized medicine and the integration of artificial intelligence for optimizing treatment strategies offer promising directions for improving outcomes in patients with IgAN. The review concludes by emphasizing the need for continued research to better understand IgAN's pathogenesis and further develop innovative therapies that prevent progression to end-stage kidney disease.

Cultivated land is the basis of food production. Reasonable planning of cultivated land is crucial to increase food production, optimize resource utilization and maximize the economy. With the acceleration of global population growth and urbanization, cultivated land resources are becoming increasingly tense. Therefore, scientific planning of cultivated land can improve land use efficiency, ensure food supply, and prevent soil degradation and environmental pollution. In addition, reasonable cultivated land planning can also promote the improvement of agricultural productivity, increase farmers ' income, and provide stable food security for national economic development. Through literature review, this paper discusses the application of robust optimization technology in crop planning, and establishes a mathematical model based on actual data, aiming to provide sustainable farming schemes for rural agriculture. In the design process, we comprehensively considered the economic benefits, land fertility and crop diversity, and finally came up with a set of farming programs that can meet the needs of farmers and contribute to ecological protection to promote the healthy development of the rural economy.

OBJECTIVE

To evaluate the renal function and percent improvement in the antero-posterior diameter (PI-APD) outcome in different operation age for ureteropelvic junction obstruction (UPJO) with antenatal diagnosed .

PATIENTS AND METHODS

* A retrospective review cases that under 3 years who underwent pyeloplasty from 2011 to 2015 for UPJO with prenatally diagnosed in single centre. All patients were underwent and recorded renal ultrasonography and diethylene triamine pentaacetic acid (DTPA) scans for preoperative and postoperative. Anderson-Hynes dismembered pyeloplasty was carried out for all cases.

* Records were evaluated with respect to age at operation, preoperative antero-posterior diameter (APD),  preoperative split renal function（SRF）and postoperative course.

*Considered the PI-APD>40% or SRF was improved by >5% whose underwent pyeloplasty is improvement, analyzed improvement and un-improvement patients' age.

RESULTS

* In the period 2011 to 2015, 49 cases aged <3 years of prenatally diagnosed UPJO underwent dismembered pyeloplasty with clinic date completion in our centre.

* Of these patients, male (n=46, 94%) and female (n=3, 6%) were in our study, from 1 month to 36 month, with the median age of 3.8 months. Follow-up time ranged 6 to 12 months.

* A statisitically significant different between before and after operation APD(P=0.000) or SRF(P=0.046) under the Wilcoxon rank sum Tests. There was a significant negative correlation between the age of surgery and PI-APD (R=-0.340, P=0.017). And significant negative correlation were observed between the age and improvement SRF>5% ( R=-0.382, P=0.007)

* 5 patients underwent redo double –J stent cathelerization because hydronephrosis aggravate in follow-up. The overall success rate was 89.7%.

CONCLUSIONS

* The age of pyeloplasty maybe a factor to outcome for spilt renal functional and PI-APD.

* Once a UPJO patient has a operation indication , allow appropriate early time for under surgery intervence is necessary.

This paper examines how Australia's climate laws, litigation approach, and market mechanisms work together to mitigate climate change impacts. The paper explores the role of international climate law in shaping domestic policies and emphasizes the growing trend of climate change litigation in Australia, which has had significant effects on environmental law. Additionally, the paper discusses the market mechanisms, including taxation, trading schemes, and subsidies, that drive Australia's efforts to reduce greenhouse gas emissions. In conclusion, a multidisciplinary approach combining legal frameworks, litigation, and market-based policies is essential for addressing climate change, ensuring sustainable development, and enhancing Australia's resilience to future climate impacts.
